Low-Intensity Extracorporeal Shockwave Therapy (LI-ESWT) has been studied as a treatment approach for erectile dysfunction, particularly vasculogenic ED.
Randomized clinical trials and systematic reviews have reported improvements in erectile-function scores in some patients treated with LI-ESWT. However, study protocols, treatment parameters and patient populations vary, and the overall certainty of the evidence remains limited. Recent reviews indicate that LI-ESWT may provide a small or modest improvement in erectile function for some men, while the clinical significance and durability of these improvements continue to be studied.
For this reason, treatment should be considered on an individualized basis following a clinical assessment. Patients should understand that results vary and that LI-ESWT is not appropriate for every cause or severity of erectile dysfunction.

Cochrane Review, 2025 — Low-intensity shockwave therapy for erectile dysfunction. Review of 21 randomized controlled trials involving 1,357 participants. The authors found a possible small improvement in erectile function compared with sham treatment, but rated the certainty of the evidence as low.
Systematic Review and Meta-analysis, 2026 — Effects of Low-Intensity Shock Wave Therapy on Erectile Function and Penile Vascular Parameters. Twelve randomized studies were included. Improvements were found in some IIEF erectile-function scores, although the authors concluded that clinical benefit remains limited for many patients and further research is required.
Systematic Review and Meta-analysis, 2026 — A Close Look at the Evidence on Low-Intensity Shockwave Therapy in Erectile Dysfunction. Nineteen randomized controlled trials were reviewed. Although statistically significant improvements were reported, many studies did not reach the threshold for a clearly meaningful clinical improvement, and optimal treatment protocols remain uncertain.
Updated Meta-analysis of Randomized Trials, 2025. Twelve randomized controlled trials involving 882 men with vasculogenic ED showed improvement in erectile-function measures compared with sham treatment.
Systematic Review and Meta-analysis, 2022. Evidence suggested that LI-ESWT may improve erectile-function measures, particularly in some patients with mild-to-moderate ED.
American Urological Association Erectile Dysfunction Guideline. The AUA considers low-intensity extracorporeal shockwave therapy investigational, reflecting continuing uncertainty regarding long-term effectiveness and optimal patient selection.
